JOB SUMMARY:
Responsible for providing comprehensive clinical nutrition care services such as, inpatient visitation, patient nutritional assessment, developing individualized nutrition plans, instruct & educate patients & patient families regarding disease specific nutritional aspects. The candidate possesses knowledge of clinical nutrition care standards / principals & professional knowledge of tertiary care hospital based clinical nutrition. The incumbent is to ensure that documentation related to total nutritional care is appropriately followed in a timely manner in line with systems, processes and nutrition policies. The individual is trained to be able to operate HI (hospital information system) independently and flexible to undertake and manage clinical nutrition operational activities during un-usual situation / circumstances. Able to undertake operational assignments as assigned and perform on call nutritionist duty as per established schedule. The individual is also responsible for handling of nutrition consults of inpatients / out patients. Ability to maintain professional coordination with faculty, physician / nursing management / Clinical Nutrition Department support staff and other allied heath care providers.
Tasks & Accountabilities
- Responsible for the visitation, assessment, intervention and multidisciplinary liaison relating to assigned in-patients areas within 24 hours of patient admission, (credentialing criteria is applicable to perform clinical nutrition & dietetic consults,
- Responsible for patients (referred by physician) nutrition care priorities to identify acuity level and to complete nutrition assessment.
- The patient nutrition assessment is documented in Patient Responsible to regularly participate in the consultant, physician ward rounds in-accordance with TIH faculty round schedule.
- Responsible to develop disease specific nutrition care handouts in consultation with faculty chair. Responsible to conduct outpatient multi specialty nutrition care clinics as approved & directed by HOD. It is to ensure that outpatient nutrition care clinics are not conducted by junior nutrition staff or non-credential nutrition staff.
- Responsible to undertake effective management of outpatients clinics (new, follow up, patients) as per assigned schedule or when assigned and to ensure that work is completed timely and appropriate tools / systems are followed and records are maintained
- Responsible to ensure that inpatients are provided healthy balanced diet, nutritionally vulnerable and comply hospital based clinical nutrition standards
- Responsible to ensure that nutrition screening tool is developed, approved and applied to patients where essential and screening evaluation is done e.g. patient poor appetites, increase risk of malnutrition, allergy parameters
- Responsible for supervising, assisting & monitoring Dietetic trainee's day to day operational activities and ensuring smooth delivery of nutritional care to patients
- Responsible to ensure that patient therapeutic / non therapeutic diet is in accordance with physician diet order entry to comply patient individual nutritional assessment, patient likes & dislikes & patient food allergies are noted / taken care of
- Responsible to provide disease specific nutritional education / guidelines to patients & families regarding nutritional care and appropriate medical nutrition therapy.
- Responsible to conduct monthly clinical nutrition training for onboard diet technicians, dietitians, interns & res dents. SD patient meals, nursing and other allied health care staff. Responsible to maintain records
- Responsible to plan and budget annual trainee dietitian / intern program and insure that TIH budget approved program is implemented as directed by the HOD
- Responsible to conduct & coordinate post meals ward round in assigned areas and ensure that »patient Meals" are in compliance with physician diet order
- Responsible to act within professional and ethical standards outlined by Indus Hospital, code of professional conduct, code of ethics and the organizational code of conduct
- Responsible to resolve patient complaints and visit patient upon receiving a complaint and ensure maximum satisfaction with in available resources
- Responsible to undertake clinical nutrition schedule, during hospital major emergency situation and flexible to undertake supporting role where applicable / required or as directed by the management
- Responsible for the planning & safe management of inpatients Enteral Nutrition / TPN in accordance with physician recommendation.
- Responsible for thorough evaluation of all nutrition supplements - products prior to patient consumption. Product evaluation in consultation with relevant faculty, management of pharmacy and SC
